Thanks for your detailed bug report.

I have the feeling that the behaviour inside Mapserver changed for things
related to the labelcache drawing. I couldn't find any information in the
changelog about it, but if I comment the $msMapObj->drawLabelCache() line in
ServerImages, only one label is drawn.

I couldn't find other side effects by commenting this, so I will fix it this way
for now (I added a comment in the source about this issue).
