well, I would say the problem is much more fundamental, as it doesn't seem
to come from Cartoweb itself but from the externals application, like
mapserver or apache, maybe the php_mapscript drivers.
Did you compile them or were they part of packages ?
map cache is totaly disabled if you have set profile = custom and
noMapResultCache = true, which you have already done.
what you could try is to disable Smarty caching too, smartyCompileCheck =
true
also, do you use Cartoweb in direct access or with soap between two servers
?
