okay, so if you didnt modify anything in Cartoweb, I would say something is screwed in your webserver, maybe the apache configuration or so. 

do you have the same problem when you access the application localy directly on the server ?

Where can I see these parameters about the ?mode=image ? 
What's the file? 
For the exportPdf, i see that we have the same problem, it doesn't work normally.... 
There is a file for managing these plugins?

mmm, your application behave quite strangly. 

what did you modify to have the ?mode=image open in a new windows ? because this seems to be the source of the problem 

on a side note, the exportPdf seems to have the same kind of weird behaviour 

and a ?reset_session break your map/keymap

Normally it must to open in a new map. 
In my application, I have a message error in a new map....
