Removed second "sample application Link from:
chameleon\ms4w\Apache\htdocs\chameleon-1.99.pkg.html

Created new sample section called, CWC2 Service Instance in:
chameleon\samples\htdocs\index.phtml

Paul, Question do you think it is important to add other examples of the service
instance for beta 3?
