[Chameleon] Error Rendering Map Image

I resolved my earlier issues with the blank browser pages.  I had not
installed the Proj .dll properly.  I am having a different problem with
the default app
(http://localhost/chameleon/cwc2/cwc2.php?service=VCG&version=0.1.0&requ
est=GetApplication).

I have to clear my browser's cookies, temp files, and history, in order
to display the default application.  Even after the map is displayed, as
soon as I navigate or zoom anywhere in the map I get the following
message in the map object window: "Error Rendering Map Image - a fatal
error occurred when drawing the map".

This is accompanied by the following Apache error log message:

Also, many of the entries in the Apache access log indicate a code 304
rather than a successful 200.
