[ka-Map-users] scalebar update

Thanks Paul,
now it works.

I've posted an updated index.html
now it's validated as valid HTML 4.01 Transitional.

I've moved onresize function from body tag to myOnLoad() function.
I've put some alt attribute missing and I've moved the form tag.
I've set encoding

it seems to me working on Safari and Firefox. No IE tests yet.
