I find that I can zoom in on my dataset, but as soon as I pan or zoom to full extent, the map goes blank. I even lose the ROSA controls sometimes. I found that if I removed the INPUT_COORD=0%2C0%3B0%2C0 (which is interpreted as INPUT_COORD=0,0;0,0) the map would load fine. I'm not sure if there's something missing from my mapfile, or if the PHP code generated by GMapFactory has some kind of bug (I'm also using the latest release). Also, I didn't have this problem until I altered my mapfile with new max extents, and started using my layers in PostGIS. I hope someone can offer a better solution.
