I set display_error = On and get the following:

Warning: [MapServer Error]: msSearchDiskTree(): Unable to open spatial index
for F:/maplab-2.1-RC1/htdocs///tutorial/./data/grid.qix. In most cases you
can safely ignore this message, otherwise check file names and permissions.
in F:\maplab-2.1-RC1\htdocs\mapedit\preview.php on line 465

Warning: [MapServer Error]: msSearchDiskTree(): Unable to open spatial index
for F:/maplab-2.1-RC1/htdocs///tutorial/./data/province.qix. In most cases
you can safely ignore this message, otherwise check file names and
permissions. in F:\maplab-2.1-RC1\htdocs\mapedit\preview.php on line 465

Fatal error: Call to a member function on a non-object in
F:\maplab-2.1-RC1\htdocs\mapedit\preview.php(472) : eval()'d code on line 1

-----------------

I cannot find any qix files in the maplab-2.1-RC1 directory tree. Am I
missing something?

Are you getting any php error messages? If you have display_errors
turned off, they may be getting written to a file as directed by your
php.ini file instead of showing up in your browser.
