Thank you Gilles. I checked the path for legend template and it was correct. However this led me to look at the HTML source of the template and I saw that the relative paths for the icons all began with a period ("./foo/bar/") - this *nix convention breaks paths in Windows. When I removed the periods from the relative paths in the HTML document the icons could be found and the legens displayed correctly. Thank you for your suggestion. If there is a path for the option called "Template" check if this is the one you enter. If not, leave the box blank.
