Thank you Gilles. I checked the path for legend template and it was correct.

However this led me to look at the HTML source of the template and I saw
that the relative paths for the icons all began with a period ("./foo/bar/")
- this *nix convention breaks paths in Windows. When I removed the periods
from the relative paths in the HTML document the icons could be found and
the legens displayed correctly. 

Thank you for your suggestion. Hopefully this will help someone else too.

Hi,

You should check in the Object Browser in MapEdit the object Legend. If
there is a path for the option called "Template" check if this is the 
one you enter.
If not, leave the box blank.
