The reason may be that some dll's needed by php_mapscriopt_36.dll were not
found. Are you using php/mascript downloaded from
www2.dmsolutiu0ons.ca/mapserver/dl/  site ? If it is the case the zip files
conatins several dll's that should be installed in your system directory
(please refer to the README).

 Make sure also to use php_mapscript dll that was built for the version of
PHP you are running.

 I also recommend using an utility tool called
http://www.dependencywalker.com/ that will show you if depending dlls are
missing.

 PS : please use the mapserver user list for these issues since It will be
seen by more people and will increase the chance of getting an answer. It
will also benefit all the community when a problem is fixed.
