I put a question about this on the list about a year ago 
(http://lists.dmsolutions.ca/pipermail/mapserver-users/2002-March/001113.html).
There has been a posting from Steve Lime 
(http://lists.dmsolutions.ca/pipermail/mapserver-users/2002-August/174115.html), 
  and Frank Warmerdam put an extensive review in BugZilla: 
(http://mapserver.gis.umn.edu/bugs/show_bug.cgi?id=15 , end of 2001).

It seems to be a tough problem, and AFAIK it has not been solved yet.
