There's one at http://www.mapsherpa.com/make_a_map/ ... there are both
HTML versions and Flash versions of the same app.

A few quick notes:
- The Flash version requires the Flash 6 plugin, you'll get an empty
window if you use Flash 5.
- The HTML version has a few glitches when you try to place points on
the map... should be fixed soon.
