Chameleon Sample Application is separate from the Chameleon pack.
This should have been placed in the ms4w download page. But you can 
still find it under the Download Chameleon Section on Maptools. The only 
thing you may have to do is set up a web Alias file. (see Attachment, 
this how my Chameleon is setup).

Good suggestion, I will place this in ms4w maptools dugzilla...
