Try downloading/extracting one of the "MapServer Itasca Demo 
Application" or "PHP/MapScript Sample Application" packages from 
http://www.maptools.org/ms4w/index.phtml?page=downloads.html and see how 
those applications work.

Or:

- download a previous conference workshop on MapServer (such as 
http://mapserver.gis.umn.edu/community/conferences/MUM3/workshop/)
