I have taken the time to install mssql2008-express in a virtual machine, 
load data and connect to MapServer (whoa my brain hurts!) and I now 
verify this same error with the latest MS4W release containing MapServer 
5.2.1

I have therefore filed a MapServer ticket for this: 
http://trac.osgeo.org/mapserver/ticket/2823

..and an associated MS4W ticket:

If you want to follow along the progress of this problem please add 
yourselves to one of those tickets and join in the fun.

Also, as I mentioned in the MS4W 2.3.1 announcement, anyone can file a 
ticket, and in fact this is the encouraged way of reporting issues.
