Update: the gdal-sde plugin for 9.2 was built in debug mode causing this 
error.  I was testing with sde 9.1 so I missed this.  Thanks to Howard 
Butler I have replaced the 9.2 dlls in ms4w 2.2.7 (you'll have to 
download it again now and give it a try).
