MS4W (and it's Apache) is pre-configured for the following mapfile settings:

   IMAGEPATH "/ms4w/tmp/ms_tmp/"
   IMAGEURL "/ms_tmp/"

Therefore you should make sure that your application uses the "/ms_tmp/" 
  URL for accessing map images.
