I think the gamma parameter for omerc isn't yet supported in Proj.4.

Hermann Peifer gave the solution to use the +rot_conv flag instead.

Indeed, if I'm not mistaken, in PROJ only an alpha parameter is supported. 
But if you use +rot_conv, then the alpha is taken as a gamma.

In Evenden's libproj software both alpha and gamma can be given 
independently.

By the way: is Kertau on the Everest 1930 ellipsoid or on the Modified 
Everest 1830 ellipsoid?
