[Proj] NTv2 nested grid issues (Issue #177)

Hi

I've just struck the problem with NTv2 grids with nested subgrids of arbitrary depth.  This was noted in issue #177 from 18 months ago, though I think the proposed fix there is not completely correct.

I've submitted a patch against this issue.  Is there any chance this could be include in 4.9 please?
