[Proj] Proj.4 Trac instance now dead

The old Trac instance has been retired and it now redirects to http://proj4.org.

If you have documentation changes, additions, or bugs, please make tickets or pull requests to the Proj.4 repository at https://github.com/OSGeo/proj.4 The website is refreshed from the GitHub repository when changes are made.

Special thanks to Kristian Evers for leading and coordinating the efforts of the proj4.org contributors. It has been neat to watch the individual projections pages on http://proj4.org/projections/index.html fill in with content (like http://proj4.org/projections/merc.html for example).
