[Chameleon] Query Widget Customizing Results

hello,

Today, i custom Query Widget. I wand just .tab information without display 
SHAPE_INDEX, TILE_INDEX and LAYER_INDEX.

I can do that but i want to get the similar popup display like Query Widget. 
What kind of files of chameleon i can use ?
