[Chameleon] CWC2 service instance query result template

Oups!
I don't know if it's the case for everyone using Chameleon but in my ms4w
installation, my query_utils.php is in the chameleon\htdocs\widgets file and
not in the chameleon\htdocs file. I think that should also be corrected in
the wiki FAQ page
(http://chameleon.maptools.org/tiki/tiki-view_faq.php?faqId=4).

Excellent point ... that was overlooked when copied into the tiki 
editor.  I will update the tiki shortly.
