[Chameleon] PHP/MapServer/PROJ issue, Invalid or no access totemplate/context

Maciej,

I was having the same problem and what I found was that this error was
being caused because the projection that was being request wasn't
actually found in the epsg file. I grabbed an updated epsg file from one
of our other MapServer installs and this resolved the problem. I've
attached epsg file I'm using. This may help.
