[FWTools] GDALOpen craches the second time it is called....

Sjur,

The obvious conclusion is that the CreateTransformedGrid() function is
doing something wrong, possibly corrupting the heap.  So i don't think it
is fair to ask us to ignore what CreateTransformedGrid() does but still
expect suggestions on what might be causing the problem.
