[FWTools] Big NTv2-Grid-File causes problem

Ernst,

I skimmed the proj.4 code and I didn't see any obvious limitation on
NTv2 file size other than the 2GB limit on files accessed using stdio.
How big is the file?  What error message do you get?  Why did you mention
1000000 specifically?
