[ka-Map-users] blank tiles on ms4w

Hi.

I have this ka-map app working super well on OSX, but I have tried
getting it to work with mw4w and I get all blank tiles.

I've checked file permissions on the cache folder and they seem fine.
The images folder gets automatically written in the cache folder,
which tells me that the write permissions are OK.

No errors in the apache logs.

I'm using almost identical map files (only paths changed).  See attached.

Any other ideas?  I'm really stumped on this.
