[maplab-users] Failure to preview in MapEdit, failure to produce map in GMAP

I am running msw4_1.0.2 with maplab and chameleon installed.  Tutorial map 
works perfectly.  I have edited my mapfile to resemble its format as closely 
as I think it should be.  However I get these errors with mapedit preview:

Mapfile resides in its own folder in the same manner as the tutorial map 
file with data and etc folders at the same directory level as the tutorial 
folder.  I have tried to make it all exactly the same as the tutorial folder 
is set up.

Here is the map file.  I have no idea what I could be doing wrong:

What am I missing?  This is the closest I have gotten maplab to work, i.e. 
functioning tutorial map.  It would be really great if I could get this 
finally working.

The only GIS data in the map is a single shape file.  If I can't get that to 
work, I don't what I can get to work.
