1. Check that your raster image is georeferenced (for instance, does
it have a world file?). Take a look at
http://mapserver.gis.umn.edu/docs/howto/raster_data

2. Maplab's built in querying is limited to returning all attribute
data for features within a given tolerance of a point or area. If you
want something else, you will have to modify the generated php code.

3. No, that's not possible without modifying the code. You may want to
consider using chameleon (see maptools.org) which also uses mapserver
but includes some widgets that may do what you want. You create
applications in chameleon by writing an html template.
