Hi, try changing your links in the pkg.html file to mapserv_40.exe? instead of mapserv_40? <a href="/cgi-bin/mapserv_40?map=/ms4w/apps/gmap/htdocs/gmap75_wms.map&"> should be: <a href="/cgi-bin/mapserv_40.exe?map=/ms4w/apps/gmap/htdocs/gmap75_wms.map&"> Also verify if the Mapserver CGI you have in the cgi-bin directory is named "mapserv_40.exe" and not e.g. mapserv_41.exe or mapserv.exe.
