Hmm.  A quick test could be to try to run mapserv.exe at the command 
prompt, and see if any error or missing dll is displayed:

   c:\ms4w\Apache\cgi-bin\mapserv.exe

should output:

   This script can only be used to decode form results and
   should be initiated as a CGI process via a httpd server.
