Hi Rob,

This error could possibly be due to changes in MapServer 5.0 in the way 
CGI variables are passed (your CGI template may be setup for MapServer 
<5.0).  There is a section on the documentation on this 
(http://www.mapserver.org/cgi/controls.html#changing-map-file-parameters-via-a-form-or-a-url) 
or you can also search the mailing list archives for a solution, as this 
is a common problem (http://n2.nabble.com/MapServer-f1969210.html).

I hope that gets you in the right direction.
