Good question.  I've tried this locally with MS4W and a test dataset
(http://www.aeroplanner.com/cadrg/Samples/CADRG_L22.zip), trying to get
MapServer to read the A.TOC file (no problem with gdalinfo, however
problem with MapServer).  Of course no problem to view the individual
NITF files with MapServer, but I am not sure either how to load this
A.TOC and its subsets automagically into a DATA statement in the mapfile.
