[Proj] Help with custom projection

I have a user who is using a custom projection and is in need of some assistance. I thought I would forward this to the experts. following is his e-mail.

Dear friends
 
I use Garmin Vista C device. In Croatia, we mainly use Gauss-Krueger (UTM) 
grid for mapping. To get the coordinates in this way I use this parameters 
in my device:
 
User UTM grid:
 
Longitude origin: E 15°00,0000
Scale factor: 0,99990000
False Easting: +5500000
False northing: 0
 
User Map datum:
 
DX=   +675
DY =  -205
DZ=   +475
DA=  +740
DF=  +0,00010040
Bessel elipsoid
 
I have been trying to craeate my own custom projection this way:
 
# Croatia/ Gauss Kruger zone 15
<????????> +proj=tmerc +lat_0=0 +lon_0=15 +k=0.9999000+x_0=5500000 +y_0=0 +ellps=bessel+towgs84=675,-205,475,740,0.000100400,0,0 +units=m +no_defs 
no_defs <>
 
but haven't got the wanted result.
 
Can you help me?
