[Shapelib] spatial index support - idx file

It turns out that the idx file they use is an RTree(?) index file.  The
file header indicates "Gist data file  rt_point_ext".

Ah.  Well, I would assume it is an index format specific to their
software, so you will need to talk to them about it.
