Bart, the sample is intended to be installed in /ms4w/apps/ in parallel to chameleon. It looks like you have installed it in /www/MapServer. You will need to edit the configuration file of chameleon to make sure that the various paths are correct. You will need to edit the index.phtml in the chameleon_sample to reflect the correct path to chameleon.
