[ka-Map-users] Example map file?

Can someone provide me with a good example of a map file? The one I've been dealing with is legacy and has all kinds of stuff commented out and looks like it has been hacked several times over. My particular concern is that the GROUP field is commented out. Also this map file is fairly old so I'm not sure if there is additional new fields supported besides the METADATA which I added based on reading the init.php header.
