I had the same problem in Windows and to make it work I had to put shp2img in the cgi-bin directory along with global.map, and navigate there from the command line to run it. Then it worked. Ditto with other script calls like the ones that return the extent from the shapefile.
This even though all the path statements had been entered. I can only guess that's it's a peculiarity of Windows but it doesn't affect the ability to run ka-map from mapfiles in the usual directories, only these little command-line calls.

Sahar,
this is because either libmap.dll is not on your computer (reinstall
ms4w) or it is not in your path (add the directory containing
libmap.dll to your path).
It sounds like you are having some very basic problems working with
ms4w and haven't even got to the kaMap bit. I highly recommend that
you ask any further questions on the ms4w users mailing list as it is
likely they will have better advice for you on how to get this stuff
working
